Facilitator/ Lead area of the AHP Skills Maximisation toolkit – Stage 1: clarify the journey  

Aim: This facilitator/ lead section of the toolkit aims to help you facilitate Stage 1: Clarify the Journey using a workshop model.

Workshop 1

Stage 1: Clarify the Journey 

Approximately 4 hours 

The following steps are covered in workshop 1:

1.1 Identify the Journey 

In this step you consider and agree the service user journey you and your participants/ team will focus on. 

1.2 Understand and Map the Journey  

In this step you and your participants map the service user journey. This step is about understanding the journey from the user’s perspective (and their family/ carers). You will create a map.  

 

1.3 Agree the Journey

In this step you and your participants jointly agree the final process map of your service user journey.

In this step you gather perspectives from others.

  • Will you need breaks added to the workshop? 
  • How will you record the outputs and actions? 

 

Next steps

Consider your next steps prior to Stage 2: Capture Uniqueness 

  • Reflect on discussions and findings from workshop 1.
  • Consider what other data, research and evidence requires to be gathered to inform any changes.
  • Consider engaging other members of the team to support data collection and information gathering.
  • Collate feedback from service users and their families.
  • Use all information gathered to inform the next stage. 
Real stories

Diet and Nutrition 

The dietitians questioned how they could meet a wide range of challenges. These were: carrying out a ‘Malnutrition Universal Screening Tool’ baseline audit, and providing training for using this tool; streamlining documentation; improving referral information and managing referrals; skill mix and team working; student training demands and issues; reducing duplication; providing more in-house education. From this list they prioritised: 

  • Education and training. 
  • Team working and skill mix.
  • The referral system.
  • Holding planning sessions. 

In prioritising some areas, they recognised that change: 

  • Takes time.
  • Is an ongoing process.
  • Needs to happen at a pace that practitioners can cope with.
