Having a future care plan at home (including in a care home) helps people of all ages get better urgent and emergency treatment and care. In hospital, a treatment escalation plan (TEP) based on the national TEP can improve continuity of care and decision making during that admission. These plans need to be recorded, shared and updated.

Effective communication in future care planning

Good conversations with people and their families about future care planning are vital. Our focus must be on these person-centred discussions as much as on developing and recording plans.

The REDMAP framework is known to be helpful when we are talking about how future care planning can help people of all ages who are living with a wide range of health conditions to manage changes in their life, health and care.

REDMAP can be used to support future care planning conversations with people at different stages of life as their health changes.

Future care planning guide (REDMAP) 2026 covers these different conversations:
•    starting future care planning - when someone is living with a disability, long term or rare condition.
•    planning ahead when someone’s health is changing.
•    talking about dying when someone with a life shortening condition is getting more unwell.

Future care planning is relevant for people wherever they live or have care and can help a wide range of people of all ages, their families and carers.

Future care planning (REDMAP) communication education toolkits

To support staff in learning how to have meaningful conversations, future care planning toolkits have been developed for tutors.

The toolkits contain all the resources needed for experienced facilitators to run interactive, future care planning workshops. They contain facilitator notes, handouts for participants, a template for writing scenarios plus some examples. Facilitators should choose scenarios that fit the context and learning needs of their participants.
