Our aim is to provide Ranfurly Care Home staff with a digital process to work in collaboration with the Care Home Nursing Support Team to have 75% of residents with Rockwood Frailty Score 7 and above to have a Future Care Plan uploaded onto Clinical Portal by November 2025.
