By January 2025, ICST East Kilbride (EK) will have created and rolled out a tool that will be used to streamline the communication pathways amongst the East Kilbride community rehab teams, and Home Care services regarding a patient’s rehab goals and rehab/medical advice. The form will also include direct contact details of the allocated therapists. By doing this, we hope to increase collaborative working towards reablement goals, thus improving patient outcomes.
