NHS Highland wheelchair referral process

This tool has been designed to demonstrate the key steps and necessary skills required to effectively complete an accurate wheelchair referral form.  This tool will support self led learning and will highlight the need for accuracy when assessing a client for a wheelchair.
Candidates will be shown the necessary tools to assess for and issue a basic, standard fleet wheelchair.  This will ensure efficient issue of the correct wheelchair first time, improving delivery time to the client.

The intention is that staff will use both the AV aid and the self assessment tool to regularly refresh and update their knowledge, maintaining skills and ensuring efficient prescribing and client service.

This collection of tools have been developed within the context of NHS Highland and NHS Western Isles.  However, these tools will also be useful for staff outside these areas, working within community prescribing of wheelchair services.
