This series of tools have been designed to demonstrate the key steps and necessary skills required to effectively complete an accurate wheelchair referral form.  These support self led learning and highlight the need for accuracy when assessing a client for a wheelchair. A brief assessment tool has been developed to accompany this, to assess level of learning following viewing this aid.This collection of tools have been developed within the context of NHS Highland and NHS Western Isles.  However, these tools will also be useful for staff outside these areas, working within community prescribing of wheelchair services.
