Cohort 25 - Digital Transformation within Primary Care Occupational Therapy: Design and Implementation of a Connect Me Pathway - Amanda Turner & Connor Bain
By May 2026, The Connect Me Patient Management system will have been designed, implemented and evaluated with user involvement a central thread running through this project. 90% of patients referred to the Primary Care Occupational Therapy service will be offered the opportunity to utilise the Connect Me platform using a digital device to interact and
engage with the service. Further to this we aim to see a 50% improvement the number of completed pre and post service defined outcome measures.
