By installing the Canary lifestyle monitoring system in selected patients under Discharge to Assess (D2A) pathway, I aim to enhance the clinical reasoning and decision-making process for the Occupational Therapists (OTs) involved in their care. This initiative is intended to improve the accuracy of care provision and ultimately reduce the overall care hours needed for these patients. The goal is to have 3 Canary systems in use by end of May 2025.
