The aim of this project is to implement the remote monitoring blood pressure program from Connect Me (Inhealthcare Connect) (IHC) for this local practice as part of NHS Shetland, with the goal of launching the service at other NHS Shetland practices. We aim to have five patients sign up within the first three months of the program.
