My NES Digital Health and Care Leadership Programme (DLP) project summary, will focus on my work relating to the commissioning and development of a website that supports the work of the West Dunbartonshire Dementia Strategy Group, specifically to provide information relating to our local dementia care pathway. This pathway includes prevention, pre-assessment activities, dementia diagnosis, post diagnosis support, through to end-of-life care.
