My project is in partnership with Technicare Solutions Ltd who have provided 20 digital wearable devices (watches) and phones and an online application to support this.
This will give patients the opportunity to take responsibility and self-manage their conditions when they have been recently discharged home from hospital with a new short term care package from Home for Me. The overall aim is for patients to return to independence and reduce the need for an ongoing long-term home care package.
