Good eSEA about a near miss event involving treatment under IV sedation. Excellent description of the event, good reflection. Event fully investigated using a “no blame” culture in the meeting leading to changes in practice to improve patient care and reduce stress on staff. Comprehensive action plan, worth studying as a template. This eSEA demonstrates the benefit of involving all staff in decision making following a significant event and good teamworking.
