Improving discharge planning in the Amputee Rehabilitation pathway in Lanarkshire

1. All environmental visits for amputees will be agreed within 3 days of amputation and performed within 5 days by May 2018 2. There will be an established Amputee Rehab Outreach Team to improve rehabilitation transition from secondary to primary care by May 2018
