NHS Education for Scotland (NES)

There are over one hundred community hospitals across Scotland, many providing care for elderly patients. They are often based in remote and rural settings, managed and staffed by GPs. They could be developed as a core resource in community-based assessment of the frail elderly. The predicted long-term outcomes would be more effective, coordinated, and more appropriately targeted assessments, leading to reduced time to source and provide appropriate care, fewer admissions and shorter hospital stays.
