“What matters to you” in ward 4C? : using person-centred approaches to improve communication

Person-centred care is a national priority in Scotland and has a distinct focus on relational aspects which include listening, enabling and treating patients with dignity, compassion and respect. One approach from the '5 Must Do With Me' elements of care is the question 'What matters to you?' This project utilised improvement methodology to test how staff in Ward 4C, a renal transplant unit in Queen Elizabeth University Hospital in NHS Greater Glasgow & Clyde, could practically apply 'What matters to you?' conversations into direct interactions with patients and families. Aim: Overall care experience will be improved and sustained at 95% or above using a conversational approach with patients and families to find out ‘What matters to them’, and how that discussion is integrated into daily care in Ward 4C by September 2016.
