Personalised planning means that support needs to be centred around what matters most to the person and their carers. Within NHS A&A it is acknowledged that care plans are not always person-centred. Work has been carried out to implement the ‘Getting to know me’ document which captures what is important to the person however there is significant variation in ensuring the meaningful information gathered within this document informs care plans and then is subsequently applied in practice. Aim: By Sept 2016 80% of long term care patients in Rowallan Ward will have a high quality personalised plan developed and implemented.
