Care and comfort rounding promotes a proactive approach to direct patient care needs and can allow for communication with the team and prompt subsequent care planning. Evidence show improved patient experience, pressure ulcer prevention and falls reduction. Ward 4D is a 30 bedded stroke and care of older adult ward. The team have already made positive interventions to increase patient facing time, using tools such as SSKIN bundle to reach 300 days with no acquired pressure ulcers and have work stations within the rooms so staff are visible to patients at all times, however the falls rate remained stable.
