Our population is living longer, often with multiple long term conditions, frailty and increasing risk factors. Older people tell us they want to be cared for at home for as long as possible (1) Anticipatory Care Planning (ACP) helps people think about their future health and social care needs, reduce unscheduled care (2) (3) and plan for changes in order to help them achieve their goal of staying at home. Aim: By February 2016, 90% of older people discharged from Intermediate Care in North West Glasgow will have an Anticipatory Care Plan.
