The existing pathway of care for Lanarkshire breast cancer patients involves discharge from the acute service following completion of treatment with an appointment to return in 1 year. In order to improve care and provide support, a service was implemented whereby all patients would be invited for a consultation with specialist breast care nurses 3 months after completion of treatment. At the consultation individual care needs would be identified, advice would be offered and, where appropriate, referral or signposting to other services to alleviate physical and emotional symptoms as a direct result of cancer treatment.
