Person-centred care is a philosophy that sees patients as equal partners in planning, developing and assessing care to make sure it is most appropriate for their needs. This involves patients and their families being at the heart of all decisions. The collection of baseline patient experience data within ward 11 indicated that patients did not always feel they were happy with the amount of information given to them or their relatives regarding their care/condition. With the introduction of a medical summary sheet the project will focus on ensuring that people are empowered to be active partners in their care and are given enough relevant information during their stay to allow this to happen.
