A new Public Services Delivery Scotland (PSD Scotland) learning series.

This is a flexible learning series to support health and care staff who wish to lead and participate in Safety Learning Reviews of Safety Events (e.g. incidents, events or complaints) where harm has occurred or could have occurred, whether physical or psychological, or where the safety and wellbeing of people have been compromised.

Safety Learning Reviews:

Safety Learning Reviews are undertaken to understand what happened, how it happened, and why it made sense for people to act as they did within the circumstances they faced. The aim is to identify opportunities to improve the system and enhance safety, rather than to attribute fault or blame.

Safety Events:

A safety event is an occurrence that resulted in harm, or had the potential to result in harm, to a patient, service user, family member, member of staff, volunteer, or visitor. This includes adverse events, near misses, and events where harm was prevented or mitigated.

Built from existing PSD Scotland resources and structured into six accessible workbooks, the course introduces key concepts, practical tools and good‑practice guidance to support safe, person‑centred learning after safety occurrences.

Each module is presented as a Completable Workbook and you will be asked to download and save your own copy of this Workbook to your own files. This will allow you to study the series at your own pace and add your own learning reflections as your work through the materials.

Learning Activities & Certificate

Rather than formally assessing competence, this programme encourages you (the learner) to reflect on your own learning journey. A Certificate of Completion acknowledges engagement with approximately 6 hours of self-directed CPD learning.

You will be invited to self-rate your confidence and understanding before and after completing the programme, this is to help you recognise areas of growth and identify opportunities for applying learning in practice.

These reflections are intended to support personal and professional development rather than provide evidence of competence.

Therefore there is no formal assessment for this Learning Series but we do require you to complete a pre and post evaluation survey, which will release the Certificate of Completion.
