GIRFE - The Team Around the Person Toolkit

Add to favourites

GIRFE is Scotland’s approach to designing and delivering public services that focus on people’s needs.  It makes sure services work well for everyone who uses them.

The approach was created with both service users and professionals from nine Health and Social Care Partnerships. It puts individuals at the centre of decision-making, bringing all professionals involved in their care together.

People in Scotland have been frustrated by:

  • having to repeat their story multiple times
  • complicated information
  • too many separate appointments

The GIRFE ‘Team Around The Person Toolkit’ will support Health and Social Care professionals across Scotland to provide support holistically to individuals in the health and social care system.

This will help ensure that people do not have to tell their story repeatedly, that they are supported to understand information about their own health and care, while being supported to navigate the system to get the support that matters to them as an individual.

The Team Around the Person includes 10 tools. These are:

  1. My Plan
  2. My Team
  3. Co-ordinator
  4. Virtual Meetings
  5. Community Hub Principles
  6. Community Connection
  7. Accessible and Useful Information
  8. Moving On (a conversation)
  9. Peer Support Training
  10. Support Bag

To help you consider the Tools in line with your own scope of practice we have broken each tool into micro learning resources.

Each Tool opens in a Microsoft Sway document and covers:

a brief introduction to the Tool.

  • the benefit the Tool can have to the person.
  • the benefit the Tool can have to the practitioner.
  • a reflective learning activity to consider how you can implement the Tool.

You can access this by clicking the image on the left or follow the link - Team Around the Person - The Tools page.

REMEMBER - If you are exploring how to embed these tools in your current service, consider how you involve the people you support. The power of GIRFE lies in co-design.
