The Primary Care Occupational Therapy (PCOT) pathway supports adults aged 16+ who are at risk of or experiencing deterioration in their ability to perform everyday tasks due to physical or mental health issues. It uses a hybrid delivery model with appointments in GP surgeries or locality hubs supported by the Connect Me remote platform.

We encourage clinicians and practitioners to explore and share the Primary Care Occupational Therapy (PCOT) Pathway, supported by the Connect Me remote platform. The resources are designed to be simple and practical, helping both you and your patients to better understand conditions, support self-management, and promote more informed care.

Upon referral, patients are enrolled manually by clinical or administrative staff who input their demographic details and contact info. Patients receive a screening questionnaire to assess their needs, which guides clinical decision-making, including appointment scheduling, referral, or provision of self-management information.

Patients who require intervention complete additional questionnaires such as:

  • Short Warwick–Edinburgh Mental Well-being Scale (SWEMWBS)
  • Short Falls Efficacy Scale (FESI)
  • CORE-10
  • Behavioural Activation for Depression Scale (BADS-SF)
  • PHQ-9
  • Canadian Occupational Performance Measure (COPM)

Clinicians monitor questionnaire responses via a digital dashboard that updates patient progress in real-time. The dashboard allows clinicians to assign tasks, log missed appointments, change communication methods (email/SMS), communicate securely with patients, and discharge patients with a summary report.

Throughout the pathway, patients receive reminder notifications if questionnaires are incomplete, and clinicians can take action accordingly. Patient responses generate PDF reports available in the patient’s profile and shared with clinical systems.
