The 'Learning Response Review and Improvement Tool' is intended to be used by:
The tool was developed and validated by NHS Education for Scotland (NES) in partnership with the NHS England Patient Safety Team and the Health Services Safety Investigation Body. The latter two organisations also recommend its use as part of Oversight processes for safety investigation and learning. The Tool is embedded in NES training programmes related to safety learning reviews. It has also been adapted and recommended for use in the New Zealand health system.

The Tool has also been adapted as a brief checklist/cognitive aid for those who Chair, Moderate, or Facilitate team learning meetings (e.g. hospital Mortality & Morbidity or significant event reviews in primary care) where care quality and safety issues are discussed and analysed.

To increase the Tool’s utility, plans are underway for researchers to critically review selected high-profile maternity safety investigation reports in 4 different countries.

The Learning Response Review and Improvement Tool (LRRIT) is increasingly being used to support the review of Patient Safety Incident Response Framework (PSIRF) learning response reports (NHS England) and other safety learning approaches across the UK home countries (NHS Scotland) and internationally.
However, in its original form, LRRIT requires evaluators not only to exercise judgement about the quality of a learning response, but also to interpret what individual LRRIT questions and evidence levels are asking of them. This interpretative step can introduce unnecessary variation, with different evaluators applying their professional judgement to subtly different understandings of the same criteria.
This scoring rubric was developed to address that interpretative ambiguity. It provides explicit, shared criteria that clarify what is being evaluated, allowing professional judgement to be applied to the same reference points across evaluators.
