The Core Home Monitoring Pathway enables patients to monitor one or more of their blood pressure, heart rate, spO2, and body weight, and to send in readings on a pre-arranged schedule according to clinical needs. 

The pathway is available to patients on four communication channels:  

  • Online portal and smartphone app – A link will be provided to the questionnaire on the online portal via email. Alternatively, the patient can download the My Inhealthcare app to submit and view their readings.
  • Online portal and SMS – A link will be provided to the questionnaire on the online portal via SMS from NHS Scotland. 
  • SMS – Patients will receive SMS communications from NHS Scotland and will be able to respond directly to those texts with their readings.  
  • Automated phone call – Patient will receive phone calls and will be able to submit their readings using key tones.  

The clinician can choose any combination of the four core readings (blood pressure, heart rate, spO2, and body weight) and specify how long the monitoring period should be, the clinician can also change the combination at any time. 

 Thresholds for readings are set by the clinician based on the individual’s condition and severity. 

 Readings submitted by the patient are displayed on a dashboard in the clinical front end of the Inhealthcare portal, the data (over time) is also displayed in chart form and can be viewed by selecting the view data tab within the patient record.  

The readings on the dashboard are colour coded and change based on the thresholds set by the clinician. 

 There is an option to select a PDF output report and choose the frequency of reporting, these will be available in SCI-Store. 

Ad-hoc messaging service option 

Depending on whether ad hoc messaging was enabled during completion of the referral task, the clinician can message the patient. If one-way messaging is selected, the patient will be unable to message back. If two-way messaging is selected, the clinician and patient can message back and forth, however this must be initiated by the clinician (patients cannot send messages unless asked to do so). Two-way messaging is not available on the SMS only option. 

If the automated phone call service option is selected upon completion of the referral task, ad-hoc messaging will not be an option. 

Alerts 

There are two fixed safety alerts built into the pathway. The clinician will be notified via a task in the portal if a patient’s systolic blood pressure rises above 200 and if their heart rate drops below 40. There is an optional for the clinician to choose an alert if the daily weight readings increase by 2kg over 48 hours.
