The pathway is for anyone with symptoms of a musculoskeletal foot and ankle condition. Clinical staff from the orthopaedic, orthotic, and podiatry services within a local health board use it to support and make decisions about the patient’s care journey through those services.

MSK Podiatry Service

We encourage clinicians and practitioners to explore and share the MSK Podiatry Service Pathway, supported by the Connect Me remote platform. The resources are designed to be simple and practical, helping both you and your patients to better understand their condition, support self-management, and promote more informed care.

Upon referral, patients will receive an initial assessment questionnaire with questions about their symptoms, the answers to which will support clinical staff to decide what level of care they receive. Depending on their route through the service, and whether they have received satisfactory relief of their symptoms, they may receive the same symptom questionnaire several times. 

After each appointment or medical intervention, patients will receive a short questionnaire asking if they are satisfied with the service received and if their symptoms have improved. They will receive the same questionnaire upon discharge from the service; when they have achieved satisfactory relief of their symptoms and they require no further appointments or interventions. 

Six months post-discharge from the service, patients will receive a follow-up questionnaire with questions about their symptoms and asking if they are satisfied with the service received. 

Throughout the patient’s journey through the service, clinical staff will be able to view the patient’s answers to make decisions about patient care and to form the basis of a shared discussion at appointments. They will also be able to add relevant diagnostic codes to the patient’s profile. 

After the patient has completed the service, a PDF output report will generate, with the questions and answers from each task that the patient has responded to; relevant diagnostic codes, alongside the dates of response and entry. It will be available to view and download within the Inhealthcare platform.
