We encourage clinicians and practitioners to explore and share the Chronic Pain Management Pathway, supported by the Connect Me remote platform. The resources are designed to be simple and practical, helping both you and your patients to better understand their condition, support self-management, and promote more informed care.
Patient Inclusion Criteria
The service is intended for all patients referred to secondary care Chronic Pain Management services, eventually throughout Scotland.
The service will be available to patients who have an email (NB patients without an email address will not received details of useful resources to help with pain management).
