The Chronic Pain Management pathway has been developed to support referral to secondary care chronic pain services through self-management.  

This pathway will: 

  • Provide patient education and signposting at an early stage in the patient pathway. 
  • Collect relevant data from patients referred into secondary care chronic pain services using a standardised referral questionnaire called the Chronic Pain Management Welcome Questionnaire  and assessment. 
  • Support enhanced vetting to ensure the patient is directed to the right service. 
  • Assist shared decision making.
  • Include collection of the Scottish Core Minimum data set.
  • Standardise and improve access to chronic pain services across Scotland.
Chronic Pain Management Service

We hope clinicians and patients find the Chronic Pain Management Service  simple and easy to use, helping people better understand their condition and manage their symptoms. 

Patient Inclusion Criteria

The service is intended for all patients referred to secondary care Chronic Pain Management services, eventually throughout Scotland. 

The service will be available to patients who have an email (NB patients without an email address will not received details of useful resources to help with pain management). 


Patient Information

  • Requires the patient to complete the Chronic Pain Management Welcome Questionnaire  to proceed to secondary care chronic pain services 
  • Sends the patient a notification when the referral questionnaire is to be completed 
  • Sends the patient links to education and signposting at early stage of pathway 
  • Allows the patient to opt out  
  • Gives the patient appropriate advice and guidance depending on their symptoms and responses to the referral questionnaire 
  • Sends the patient an evaluation questionnaire when the referral questionnaire has been competed 

Although developed for patients, these resources are useful for clinicians supporting the use of the Connect Me Chronic Pain Management Service.
