The Connect Me Multiple Long-Term Conditions (LTC) Review Health Questionnaire pathway has been developed to support patients who have a diagnosis of one or more LTC(s):

  • Atrial Fibrillation
  • Coronary Heart Disease
  • Chronic Kidney Disease
  • Heart Failure
  • Hypertension
  • Peripheral Artery Disease
  • Pre-diabetes
  • Rheumatoid Arthritis
  • Stroke/TIA
  • Type 2 Diabetes
Multiple Long-Term Conditions (LTC) Review Health Questionnaire

We hope clinicians and patients find the Connect Me Multiple Long-Term Conditions (LTC) Review Health Questionnaire service simple and easy to use, helping people better understand their conditions and manage their symptoms.

Before their long-term condition review appointment, the patient will complete the health questionnaire and the answers will be collated into a PDF output report that will be available to their GP practice.

This will assist GP practices to gather information required to complete the long-term conditions review within the Primary Care and CTAC Service, and to save time during the appointment in order to take a person-centred approach; to talk to the patient about what matters to them.

Practice staff will use the PDF output report to review the alerts triggered by the patient’s answers, and to assist in the manual calculation of the patient’s CHADS2-VAsc, QRISK2, and FRAX scores before the patient’s long-term condition review appointment. This will help identify patients at risk and will save time during and after the appointment.
