To support the primary and community care management of the COVID Omicron wave, a national service to support people to self-manage has being established.  This uses an updated version of the existing national COVID-19 remote monitoring pathway, as a supportive self- management tool, with the addition of:

  • An easy to use registration service delivered by National Contact Centre. This will provide central administrative resource to onboard and provide support to people using the remote monitoring service.
  • A supply of pulse oximeters, delivered to appropriate distribution points. This will include a patient information leaflet outlining the service and asking the person to register via the 0800 number provided.
  • Further potential to connect remote monitoring with other developing community facing services such as virtual wards, intermediate care and provision of antiviral therapy.
COVID-19 Self-Management Information for Clinicians

This short video is for clinicians to explain the COVID-19 Self-Management Service. This automated service works via text message or phone call and is for patients who do not require hospital admission.

National COVID-19 Self-Management Service

We hope clinicians and patients find the National COVID-19 Self-Management Service simple and easy to use, improving patient’s confidence with technology and self-management of COVID-19 in the community setting for people with COVID-19 symptoms, and who may be at risk of deteriorating.

The COVID at Home - Remote Monitoring Pathway Briefing document provides clinicians with details of the COVID at Home Remote Monitoring Pathway and Operational Definitions.

The National COVID-19 Self-Management Service Organisational Guidance supports clinicians to understand the COVID-19 self-managed pathway and who it is and is not suitable for. It also includes the pathway rationale, explains how it works, what patients are advised and why, and where to get the equipment.

The slide deck and draft letter, which you can download below, is designed as a resource for health boards and services to support engagement with the COVID-19 Self-Management Service. Please use or amend as required for any specific audience requirements or local process.


The COVID Remote Monitoring Clinician Guidelines operational guide supports clinicians to understand what is COVID remote monitoring, who it is suitable for and what they need to do as a clinicians to support patients with the remote monitoring of COVID in the community setting.


The COVID-19 Self-Management Additional Clinician Decision Support resource with additional factors for clinicians to consider during a conversation with a patient about the COVID-19 Self-Management Service including connectivity, wellbeing and support needs, and instructions for clinicians to support patients on how to participate in the pathway.


The COVID-19 Self-Management Service guide supports clinicians to understand patient alerts including symptoms or physiological readings, the automated advice they receive and rationale.


The COVID-19 Self-Management Service Organisational Set Up guide supports clinicians to understand the process where patients are onboarded by the National Contact Centre.
