The Blood Pressure Pathway has been developed to support people to successfully submit their blood pressure readings, saving face-to-face appointments, time away from work, or other responsibilities, while helping them keep their blood pressure at safe levels. This pathway is aimed mainly within Primary Care for hypertension diagnosis, intervention, treatment and ongoing monitoring.

This pathway promotes self-management and a person-centered approach in effectively supporting and managing those citizens who have been diagnosed with hypertension.

NHS Scotland is scaling up the use of remote monitoring to manage blood pressure in line with Scottish Government priority for 2020/23.

Blood Pressure Service

We hope clinicians and patients find the Connect Me Blood Pressure Service simple and easy to use, helping people better understand their blood pressure and manage their symptoms.  

The automated monitoring system reviews responses and readings providing feedback and advice specific to the patient helping them manage their symptoms.



The Blood Pressure [Primary Care] Clinician Guide is a step-by-step instruction guide for clinicians and administrative staff on using the Connect Me Blood Pressure Pathway.


The Blood Pressure Pathway Onboarding Process guide supports clinicians to onboard patients to the Connect Me Blood Pressure Service via the automated monitoring service.


The patient Blood Pressure Pathway Shared Management Plan (lower readings) supports clinicians to understand readings, information and the automated advice patients receive.


The patient  Blood Pressure Pathway Shared Management Plan (standard readings) supports clinicians to understand readings, information and the automated advice patients receive.


Although developed for patients, these resources are useful for clinicians supporting the use of the Connect Me Blood Pressure Service via the automated monitoring service.
