The COPD self-management remote health pathway has been developed to help reduce unscheduled care admissions and increase service capacity through early identification of those most ‘at risk’ of exacerbations and hospital admissions.
The pathway provides regular tips for patients, plus appropriate education and advice depending on the patient’s responses to symptoms questions. In addition, the pathway may be suitable to promote early supported discharge.
