To improve the Health and Wellbeing Outcomes for at least 50% of homeless people/program participants (30 individual participants) in the Housing First Pathfinder Program in Aberdeen City by 2021. The target group for the SCiL program improvement chapter will be 5 program participants between April 19 and Feb 2020.
