Medicines: Care and Review (MCR) service is a refreshed service based on the Chronic Medication Service which was developed for community pharmacy teams to support people with long term conditions in managing their condition and supporting patients with their medication. This strategy document describes several commitments and suggests actions to achieve the two priorities for pharmacy in Scotland as set out in “Achieving Excellence in Pharmaceutical Care: A strategy for Scotland” (improving NHS pharmaceutical care and enabling NHS pharmaceutical care transformation) through the implementation of the MCR directions which come into force on 12th February 2021
