Aims: In one Acute Medicine of the Elderly Ward (Royal Infirmary of Edinburgh, Ward 202):
a) To improve the documentation of a delirium diagnosis (in the unitary patient notes) to 75% by August 2016
b) To improve the communication of delirium on Immediate Discharge Letters (IDL) to GP to 75% by October 2016
