Aim: By June 2019, a minimum of 30 patients with Chronic Obstructive Pulmonary Disease (COPD) in the NHS Forth Valley area will have up to-date observations recorded into their Key Information Summary (KIS). This will allow Health Care Professionals including the Scottish Ambulance Service (SAS) to adopt a Realistic Medicines approach whereby patients receive informed, person-centred care.
