Aim : Improve the design of nursing documentation on ward 3A, Raigmore Hospital by May 2019 in order to: improve completion by making it an integral part of patient care
give nurses the time to connect to what matters to each patient provide a linked-up record of care (e.g. falls, frailty, delirium, care rounding)
design at-a-glance patient information that all staff on the ward will read
