60 – 80 % of patients medicine lists controlled after admission contains one or more unintended discrepancy. With so many errors, the chance of harming patient is high.By implementing a structured way to collect information of patients medicines in use (MIU), the medicine lists will contain less discrepancies and more patients will get correct medication. Aim: Spread/implemented medicines reconciliation (MR) to all the wards in the organisation (four hospitals and five regional psychiatric centres) by 01.09.16. By 01.10.16, 80 % of the patients admitted to the hospital will have completed medicines reconciliation within 24 hours after admission.
