Cameron Community Hospital provides recovery and adaptation following acute illness. Many of these patients are frail, elderly patients with comorbidity and complex care needs. The hospital does not have acute care or on site diagnostics and thus patients may find themselves transferred to the nearby Victoria Hospital should they require a higher level of care.Whilst this is entirely appropriate and laid out in key guidance around care of older people, we were aware of a significant number of patients and families who reported poor outcomes and experience as a result of being transferred.
