Improving outcomes with medicines for homeless patients with multiple morbidities

Patients that attend the homeless practice can have a chaotic lifestyle and require input from a range of members from the multi-disciplinary team which includes psychiatry, alcohol partnerships, and social work. Patients with substance misuse problems often have long-term conditions and currently see different healthcare professionals for care, but often fail to attend for appointments to manage their long term conditions.
