Medical wards are complex environments often operating with several clinical teams and within shift patterns that present challenges for high quality care. Systems of communication are not well developed and there are opportunities for improvement in the coordination of safe, efficient, patient centered care. Structured ward rounds can reduce variability, manage human factors, improve patient outcomes and facilitate coordinated discharge planning. Aim: Implement Structured Ward and Board Rounds to: 1. increase the number of patients discharged by 20%. 2. assess and document Safety Essentials daily in 90% of patients.
