The core long term goal of the project is to reduce maternal morbidity and mortality from post partum haemorrhage (pph). The checklist is the 1stkey step in the improvement journey. 5-10% of women experience pph and rates are rising. > 50% of women admitted to ITU begin their maternity journey as low risk. They have expectations about their experience and do not anticipate complications. The checklist will improve the quality of information collected about each woman ensuring better communication and less variation. To actually reduce the number and volumes of pph’s we need to change perspective from “reactive” to “proactive“. The action plans will allow us to do this. The form is easy to read, easy to complete, easy to update and will identify “watchers”. Aim: to introduce a structured checklist and post partum haemorrhage risk assessment tool for 95% of admissions to CLU by June 2015.
