Literature states a lack of consistent, standardised medical assessment can lead to harm by omitting to gain key safety critical information from patients and family members, in order to determine the best course of action for presenting symptoms. This project prototyped a standard way of medically assessing patients in one ward, in one hospital with a view to spreading and implementing the process across NHS Greater Glasgow & Clyde Health Board. Aim: All patients should have 100% of Safety Critical Elements of a Medical Assessment completed within 4 hours of admission to the Acute Medical Unit (AMU) by September 2016.
