It is widely recognised that service users in mental health rehabilitation experience a broad range of psychotic symptoms which can impact on daily function, medication compliance, mood and motivation resulting in a reduction in their level of engagement and participation in their own care. The Mental Health Strategy 2012 - 2015 focuses on developing an outcomes approach to personal, social and clinical outcomes. This project aims to develop an outcome focused approach through 1:1 conversations within a mental health rehabilitation unit. Aim: By August 2016, 80% of patients will be engaged in outcome focused conversations with their key workers on a weekly basis.
