McMillan Ward is an assessment and rehabilitation ward within Biggart Community Hospital. Due to the age and the co-morbid conditions of the patients cared for within McMillan Ward their condition can deteriorate unpredictably. As there is no medical cover overnight, often an ANP has to make the decision to transfer the patients to an acute hospital. The absence of Treatment Escalation Plans (TEP) and Do not Attempt Cardio pulmonary Resuscitation (DNACPR) can often result in patients being transferred inappropriately and causes extreme distress to patients, relatives and staff. Aim: By the 31st June 2016, 95% of patients within McMillan Ward within Biggart hospital will be reviewed weekly using a structured ward round process.
