District nursing teams are required to communicate patient care information within this framework to their own team, locality based teams who provide weekend care and unscheduled care and also to other key colleagues. Teams also need to process information received from these sources and also organisational communication in relation to governance, safety and quality.
Project aim: To improve communication within the district nursing service in Edinburgh by introducing a daily structured handover and safety briefing by September 2016.
