Evidence suggests that methods which optimise patient flow improve quality and reduce the risk of harm. Data collected within the Rehabilitation Team identified an increasing gap between Physiotherapy and Occupational Therapy services relating to the number of patients discharged on post operative day one. Concerns were also raised amongst some therapists and nursing staff that early therapy discharge could potentially impact on quality of care and the overall patient experience. Aim:18% of primary joint replacement patients will be discharged by both Physiotherapy and Occupational Therapists on post op day 1 by December 2015 whilst ensuring that the patient experience is equitable regardless of day of discharge from therapy.
